Trim Family Resource Centre recently launched a new Social Prescribing Service to bridge the gap between healthcare and community resources. Spearheaded by Social Prescribing Link Worker Kate Fitzpatrick and funded by the HSE, the service offers a holistic approach to health and wellbeing. Instead of solely relying on medication and health services, Social Prescribing connects individuals with non-medical resources and supports within their community. Built on evidence that the key to improving health and wellbeing is to address the social determinants of health, such as improving access to services and activities and fostering inclusion, this service seeks to empower individuals to actively manage their health and wellbing, enhancing their overall quality of life.
